Treatment of Asymptomatic Smouldering Adult T-cell Leukemia-Lymphoma

Smouldering Adult T-cell leukemia-lymphoma (ATLL) represents a distinct, indolent disease state. When patients are truly asymptomatic — with no symptoms attributable to the disease — the management approach differs meaningfully from more advanced or symptomatic presentations.

This protocol addresses patients with asymptomatic smouldering ATLL: confirmed disease in the smouldering subtype, with no symptoms attributable to the disease itself. The absence of disease-related symptoms is the defining feature that shapes the first-line management strategy.

For asymptomatic smouldering ATLL, active monitoring without immediate therapeutic intervention forms the cornerstone of care. Alongside this, antimicrobial prophylaxis plays a role for all patients. Further specifics of infectious risk management, serological considerations, and the criteria guiding any escalation are detailed in the full protocol →

Asymptomatic smouldering ATLL can be managed with active monitoring without immediate therapeutic intervention [III, B].

Antimicrobial prophylaxis for opportunistic infections can be recommended for all patients [II, B].

In case of positive Strongyloides stercoralis serology, treatment can be initiated even if the patient is asymptomatic [III, B].
