Treatment of Recurrent Adrenocortical Carcinoma When the Interval from Last Surgery to Relapse Is Less Than 6 Months

First-line · Recurrent disease · Short relapse interval

This protocol addresses recurrent adrenocortical carcinoma in which the time between the last surgery or loco-regional therapy and confirmed disease recurrence is less than 6 months. An early relapse within this window indicates that further local measures alone are unlikely to be sufficient, and a different treatment strategy is warranted.

For this scenario, current evidence recommends a systemic combination regimen (EDP-M) as first-line treatment rather than repeating loco-regional measures. A separate alternative regimen is available for patients who are not candidates for EDP-M. Full regimen details and sequencing remain in the complete protocol.

The clinical targets include achievement of tumour response or stable disease and attainment of a defined therapeutic drug level. Monitoring parameters and full target criteria are detailed in the structured protocol.

  • We recommend EDP-M as first-line treatment if the time interval between last surgery/loco-regional therapy and recurrence is less than 6 months (++OO), rather than repeat loco-regional measures.
  • In patients unfit for the EDP-M regimen, (E)P-M may constitute a reasonable alternative.
