Alternatives for those patients who are unable to tolerate itraconazole or who are unable to take this agent because of drug-drug interactions or failure to absorb the drug include ketoconazole (at a dosage of 400–800 mg per day) or fluconazole (at a dosage of 400–800 mg per day), but these alternatives are less effective, and the toxicity of ketoconazole is greater than that of the other agents.
