Treatment of AML in Patients Aged 60 or Older with Therapy-Related or Myelodysplasia-Related Disease

Patients aged 60 years or older who present with therapy-related acute myeloid leukaemia (tAML) or acute myeloid leukaemia with myelodysplasia-related changes (MRC-AML) represent a distinct, high-risk group for whom standard induction approaches do not apply. This first-line protocol addresses their specific management.

Clinical scenario

The patient is aged 60 years or older and carries a confirmed diagnosis of therapy-related AML or AML with myelodysplasia-related changes. This group — which includes those with certain molecular mutation profiles — is managed under dedicated first-line guidance distinct from de-novo AML protocols.

Treatment approach

Induction uses a specific liposomal formulation recommended for this high-risk category; the complete protocol, including guidance by mutation status and consolidation pathways, is available in the full structured regimen below.

Treatment goals

The primary objective is complete remission or complete remission with incomplete haematologic recovery (CR/CRi), defined as fewer than 5% blasts in the bone marrow. Response is assessed between day 14 and day 21 after the first induction cycle.
