Descending necrotizing mediastinitis is a severe infection originating from a head and neck source — most commonly an oropharyngeal or odontogenic focus — that spreads through the fascial spaces of the neck and descends into the mediastinum.
Patients present following an oropharyngeal or odontogenic infection, typically with fever, neck and chest pain, and dyspnoea — reflecting active fascial spread downward into the thoracic compartment.
Management calls for prompt surgical intervention, including systematic debridement of affected tissue and broad opening of involved fascial spaces.
