Acute Lymphoblastic Leukemia in Lymphoblastic Lymphoma: When First-Line Therapy Does Not Achieve Complete Remission

Clinical Scenario

Lymphoblastic lymphoma (LBL) is a rare entity, accounting for 1% to 3% of non-Hodgkin lymphoma. LBL and acute lymphoblastic leukemia are distinguished by an arbitrary cut-point of 25% bone marrow infiltration. This protocol addresses patients in whom first-line treatment has not delivered the expected outcome.

First-Line Treatment Failure

The first treatment line for LBL is built on ALL-based regimens with CNS prophylaxis; for rare complications such as tumor lysis syndrome or thoracic compression, immediate intervention with corticosteroids is included. The target of that line is complete remission, achieved in 70% to 90% of patients. This next-line protocol is indicated when complete remission is not reached or is not sustained.
