Acute fatty liver of pregnancy (AFLP) is a serious obstetric emergency. When the condition is complicated by severe hepatic impairment, rapid escalation and specialist coordination are essential from the outset.
This protocol addresses women with AFLP in whom severe hepatic impairment is present. Early identification of this severity threshold is what drives the specific management pathway described in the full protocol.

Early referral to a transplant center should be made for women with AFLP with severe hepatic impairment.
Liver failure is managed with supportive care and, in some cases, N-acetylcysteine, plasma exchange, and/or eventual liver transplantation may be necessary.
