Treatment of Mild Acute Cholangitis When Grade II and Grade III Severity Criteria Are Not Met

Not all presentations of acute cholangitis carry the same risk. Identifying patients who do not fulfill the criteria for moderate (Grade II) or severe (Grade III) cholangitis at initial diagnosis is a critical step that directly shapes the treatment pathway.

This protocol addresses mild acute cholangitis (Grade I) — specifically, patients whose initial presentation does not meet the criteria for Grade III (severe) or Grade II (moderate) acute cholangitis. The severity grade is determined at the time of initial diagnosis and drives the intensity of the management approach.

Management of mild acute cholangitis typically involves antimicrobial therapy alongside general supportive care. The full structured regimen — including the specific approach to biliary drainage and etiology-directed treatment — is outlined in the complete protocol.
