When acute bacterial prostatitis is complicated by prostatic abscess cavities of 1 cm or larger in diameter, abscess size becomes a key determinant of the management strategy, and the approach extends beyond antimicrobial therapy alone.
Evidence supports that both conservative and drainage-based strategies are feasible in prostatic abscess, but cavity size matters. Larger abscesses — those reaching 1 cm or more in diameter — are generally not adequately managed with conservative treatment alone and require procedural intervention.
The protocol combines high-dose parenteral broad-spectrum antimicrobial therapy with procedural management of the abscess. Ancillary supportive measures are part of the approach. The complete regimen — including antimicrobial selection, drainage technique, and the structured transition to oral therapy — is detailed in the full protocol.
Normalisation of infection parameters and cessation of acute bacterial prostatitis symptoms.
