Acute Aortic Regurgitation in Symptomatic Patients Ineligible for Surgery

This protocol applies to symptomatic patients with acute aortic regurgitation who are not suitable candidates for conventional surgical repair or replacement, as well as patients presenting with a failed prior surgical valve.

For carefully selected patients deemed ineligible for surgery by the Heart Team, a catheter-based valve intervention performed at an experienced centre is among the options that may be considered — provided anatomical criteria are met.

Full eligibility criteria, procedural prerequisites, and the complete clinical decision algorithm are available in the structured protocol below.

Surgery represents the preferred treatment in patients with acute AR, while TAVI has only been described in individual cases or patients with a failed surgical valve (valve-in-valve).

TAVI may be considered for the treatment of severe AR in symptomatic patients ineligible for surgery according to the Heart Team, if the anatomy is suitable.
