Clinical Scenario

This protocol addresses the most severe end of the amebic colitis spectrum: fulminant disease with massive colonic involvement. The presentation requires urgent, multi-pronged management and carries significant morbidity risk.

Presenting Features

Life-threatening manifestations in this scenario may include one or more of the following:

Massive colonic involvement with perforation and peritonitis
Bowel necrosis
Toxic megacolon
Toxic appearance, fever, and hypotension
Profuse bloody diarrhea with abdominal pain and distension

Treatment Approach (partial overview)

Management combines antiparasitic therapy with fluid resuscitation, broad-spectrum antimicrobial therapy directed at peritonitis, and intensive supportive care. Life-threatening manifestations of amebic colitis include fulminant infection, which can result in massive areas of colonic involvement with perforation and peritonitis, bowel necrosis, or toxic megacolon.
Patients may be toxic in appearance, febrile, and hypotensive, with profuse bloody diarrhea, abdominal pain, distension, and signs of peritonism.
Patients with fulminant amebic colitis will additionally require fluid resuscitation, broad-spectrum antimicrobial therapy for peritonitis, intensive supportive care, and surgical intervention for bowel perforation and bowel necrosis.
Occasionally, colectomy has been necessitated by the development of toxic megacolon or extensive bowel necrosis.
