This protocol addresses acne vulgaris in patients presenting with larger acne papules or nodules — inflammatory lesions that warrant a targeted adjuvant approach beyond standard acne therapy.
An intralesional corticosteroid injection is recommended as adjuvant therapy for this presentation.
Full dosing parameters, technique details, and the complete structured regimen are available via the protocol. The primary target for this intervention is:
Resolution of injected acne cysts within 3–7 daysDOI: 10.1016/j.jaad.2023.12.017
For patients with larger acne papules or nodules, we recommend intralesional corticosteroid injections as an adjuvant therapy.
Using a lower concentration and volume of corticosteroid (eg, triamcinolone 2.5-5 mg/ml) can minimize the risks of local corticosteroid adverse events.
Although there is limited clinical trial data evaluating intralesional corticosteroid injection for acne, in a small RCT of 9 patients, most acne cysts injected with triamcinolone resolved within 3-7 days of the treatment, and those injected with triamcinolone resolved faster than those with saline control.
